About the Role: As an MDS Coordinator, you’ll play a key role in creating and maintaining care plans for our residents, ensuring that their needs are met with compassion and precision. Your work will involve:
Completing MDS and care plans in accordance with Federal guidelines.
Using care plan software to enhance our care planning process.
Collaborating with staff nurses to ensure timely completion of nursing summaries.
Monitoring residents for significant changes and following up on clinical issues.
What We’re Looking For:
A licensed Registered Nurse or LPN in Indiana with previous nursing and Long-Term Care experience.
Someone with a strong understanding of MDS and care planning.
A team player who is dedicated to improving residents' lives.
