• Up to $20,000 Sign-On Bonus
• Up to 120 hours of front-loaded Paid Time Off
• Up to $4,000 Relocation bonus

The Home Health RN Case Manager provides one on one care and skilled services to patients in their own home, contributing to the individual’s overall quality of life. Provides in home evaluation, direction and application of skilled Nursing Services to complement the patient’s overall Plan of Care. Case Management ensures the overall patient condition is cared for by providing appropriate education and resources to the patient/caregivers, allowing them to manager their conditions and remain at home.Education: ▪ Required: Graduate of an accredited nursing program or NLN approved program ▪ Preferred: Bachelor’s Degree in NursingExperience: ▪ Preferred: Previous nursing experienceSkills: ▪ Excellent verbal and written communication skills ▪ Demonstrate effective leadership abilities ▪ Exhibits valuable time management skills ▪ Strong critical thinking/problem solving skills. ▪ Flexibility and ability to work in a multi-tasking environment.Licensure/Certification/Registration: ▪ Required: RN license active in the state of Missouri
